Some people would expect healers to have a great amount of patience. That just wasn't true. Well, it wasn't true of the SunnFather Ziasla. He glanced out his window, glanced back down at his schedule and scowled. The truth was, he'd drawn the short straw of all the healers at the Monastery and was forced to take on the cethar. It wasn't that SunnRiser Ronan was a bad sort. It was just that he was smart, The SunnFather had decided. In fact, he was too smart for most of his courses. He grew bored easily and needed a challenge. That's what this whole course was for. Someone somewhere along the line suggested they make Ronan into something of a healer. If it could be done, the mixing of medicines, the gathering of medicinals, the diagnosis of disease... the care of the feeble, weak, or oppressed... all of that would keep busy mischievous hands occupied.

If... they could make it to class on time.

The cether was casually strolling along, his eyes checking suns in the sky. Ahhh not too bad, he thought with a smile. Somehow his empty pipe found its way back into his mouth, and his nose caused him to pause a few times to take the smells on the air. Hmmm smells like we will be having a good dinner this repast, as he got into viewing distance of SunnFather Ziasla's training area. In one stride he had regained that pesky limp.

He swung his leg with more zeal, in an attempt to hurry. As if he jsut realized that Sunnfather was not the most forgiving monk. When he found himself outside the Sunnfather's door he knocked, and smoothed his robes. He moved to open the door, but stopped quickly to remove the pipe and place it in his pouch. That would not do, the Father would pop his top.

He then opened the door, and greeted the scowing SunnFather. 'Serale SunnFather Ziasla, Ronan Stoutfoot reporting for his first class,' he spoke with formality. He hoped to impress the human with his zest for knowledge and attention to detail, but it may be a while before he accomplished that by the look on the SunnFather's face.

OoC I am not sure what the titles are for the younger monks. You mentioned SunnRisers, but was not sure where Ronan was falling in that hierarchy.

SunnFather Ziasla scowled once more. He had a pile of tools on or near Ronan's little study desk. One was a large pack that had a ton of tools in it, a stretcher broken down, and then a small pack. Ronan's desk contained a journal, parchment, ink, and quills. The Monk scowled again, wondering how he was going to get the student to sit still as he wrote diligently up on the chalk board.

"You are a healing steward. As such you must learn the following vocabulary... I know this is the boring part, but write it down and learn it. As we go through some of the techniques, you will need to know these terms."
The SunnFather said in a rather bored voice.

Fractures
  • Closed = a crack in the bone that doesn't cause splintering of the bone. (might not be easily detectable)
  • Compound = a break in the bone where the bone has splintered. (might erupt from the skin, normally visible)

Pulse
What is it? It's the pulsating of the veins as the heart pumps blood through them.

How can you tell? You find the 11 pulse points. What are the most common pulse points?
  • Inside the wrist
  • Either side of neck
  • Over the heart

Bandage = The wrappings used to protect a wound.

Wound = Any damage to the structure of a living organism

Bruise = The darkening of the skin, usually accompanied with tenderness or pain, of a living structure

Healing = The ability to correct any condition that makes a living organisms normal function impossible.

Tourniquet = a device made of fabric strips varying in length used to stop bleeding by wrapping around the limb. Sometimes a peg is used to slip into the wrapping and twist it until the blood stops flowing. These devices can result in the loss of the limb if the device is not removed promptly.

Stretcher = Any canvas or clothing stretched out using poles that can be used as a platform to transport a patent.



The Monk sighed, wondering if Ronan had any questions. He hated initiate classes, he really did. They were so... dull... so boring. "Thats about all the vocabulary you will need for today. Shall we start talking about the four life saving steps?" He asked, then wrote on the board again...


Four Life-Saving Steps

Restore Breathing
Stop Bleeding
Protect/Treat The Wound
Treat For Shock


The SunnFather sighed, glanced out the window, and wondered if it was time for lunch yet. Then he yawned, glanced at Ronan, and said... "Any questions?"

The young cether smiled and dipped his head bowing to the elder monk. Be it his cheerful demenour, or not paying full attention to the monk, but Ronan was not put off by the scowls. It was probably some nervous tick the older man had, Ronan had heard when humans got older they got more 'ticks'. He hopped up into the seat of the desk and went to work inspecting the contents of the desk. OOooo!! a journal, and extra parchment and a quill! He thought to himself, already diping his quill tip into the ink. He was writing his name on a peice of parchment as he realized the SunnFather had stopped talking, and was busy writting on the board.

Ronan opened the journal and began to write, a skill he had learned in Schola as a young child. He was proud of his penmanship, which was large and flowing and quick. He soon had all the writing that SunnFather had written copied into his new journal, and was adding a few flourishes. Soon the page had simple vine work up the edge of the paper. Feeling the eyes of the SunnFather on him he stopped and placed his quill into the pot.

'I am sorry SunnFather Ziasla, what was that...' He looked up at his elder with his large gray eyes, but just as he began to repeat himself Ronan spoke again. He relized that he did hear what he said, 'Oh yes, let us start. Will these two bags have...' But the man had already turned to write once more on the board.

He then quickly copied the script of Ziasla, and was once more looking over the edge of his desk to peer into the bags. When the SunnFather spoke once more asking if the cether had any questions, Ronan sat back into his seat. Yes, yes I have questions. What were in those two bags, he thought.

'Yes, uhm how do you start stopped breathing... and treat for shock, ohh yea what is shock. I mean stopping blood I can see, and treating wounds yes... but if I am not breathing... well I dont know how to start again.' Though the teacher was most certainly going to cover all he asked, and more, Ronan was excited to fill his head full of knowledge.

SunnFather Ziasla hadn't missed the first attempt at a question the Cether asked him. It was in fact the commonest question anyone normally did ask. "We'll go over the contents of your healing kit and field pack as we get to the items inside. Don't worry, we'll get there soon." He reassured Ronan, though from long term teaching, he knew it was often best to let students know. "If you sit still and patiently go through the four steps and try to learn something, we will tear into the packs as a reward." He said, somewhat understanding the nature of Ronan's double curse... youth and race.

"Now, the first thing we get to talk about is how to restore breathing. In order for a living thing to live, it has to breath. There are several reasons a person stops breathing. Sometimes they can have a wound that prevents their breath... such as a punctured lung or a something far more serious. We can't do anything about those types of injuries at this level, but there are things we can do that are just as life-affecting."
SunnFather Ziasla said, starting to sound more cheerful about all this now. "Sometimes something will get stuck in the airway.... a child will accidentally get something caught in their throat that they cannot breath around... or perhaps someone is eating, swallows too big of a piece of food, and it too gets stuck. There's a Unblocking Maneuver we can do in order to clear their airway." The SunnFather said. Again, he turned to the board.

Steps To Unblocking Airways Maneuver
  • Step 1- Ask the choking person to stand if he or she is sitting.
  • Step 2 - Place yourself slightly behind the standing victim.
  • Step 3 - Reassure the victim that you know the Heimlich maneuver and are going to help.
  • Step 4 - Place your arms around the victim's waist.
  • Step 5 - Make a fist with one hand and place your thumb toward the victim, just above his or her belly button.
  • Step 6 - Grab your fist with your other hand.
  • Step 7 - Deliver five upward squeeze-thrusts into the abdomen.
  • Step 8 - Make each squeeze-thrust strong enough to dislodge a foreign body.
  • Step 9 - Understand that your thrusts make the diaphragm move air out of the victim's lungs, creating a kind of artificial cough.
  • Step 10 - Keep a firm grip on the victim, since he or she can lose consciousness and fall to the ground if the unblocking maneuver is not effective.
  • Step 11 - Repeat the unblocking maneuver until the foreign body is expelled.


With that, The SunnFather gestured for Ronan to stand up and with that he encircled him and demonstrated the action... quietly reviewing the steps. Then, he allowed the cether to try it on him. Once that was done, the Monk had Ronan take another seat. Once he did so, he started speaking again. "Now, sometimes people just stop breathing. They could have had other issues that caused them to stop... they passed out, the air was bad, or there was no air... their body went into shock... we will talk about shock in a few minutes so dont worry, I know you don't know what that is... but you will...." He took a breath, looked stern and continued. "In these causes we restore breathing, or try to via mouth to mouth resuscitation." He said, then once more turned to the black board.




Mouth-to-Mouth Resuscitation

Mouth-to-Mouth-and-Nose Resuscitation on a Child Under Age 8 or on an Infant
  1. Place the child on a hard, flat surface.
  2. Look into the mouth and throat to ensure that the airway is clear. If an object is present, try to sweep it out with your fingers. If unsuccessful and the object is blocking the airway, apply the unblocking maneuver. If vomiting occurs, turn the child onto his or her side and sweep out the mouth with two fingers.
  3. Tilt the head back slightly to open the airway.
  4. Place your mouth tightly over the nose and mouth. Blow two quick, shallow breaths (smaller breaths than you would give to an adult). Watch for the chest to rise.
  5. Remove your mouth. Look for the chest to fall as the child exhales.
  6. Listen for the sounds of breathing. Feel for the child’s breath on your cheek. If breathing does not start on its own, repeat the procedure.



Mouth-to-Mouth Resuscitation on a Child Age 8 or Older or on an Adult
  1. Make sure the person is lying on a hard, flat surface. Look into the mouth and throat to ensure that the airway is clear. If an object is present, try to sweep it out with your fingers. Apply the unblocking maneuver if unsuccessful and the object is blocking the airway. If vomiting occurs, turn the person on his or her side and sweep out the mouth with two fingers.
  2. Tilt the head back slightly to open the airway. Put upward pressure on the jaw to pull it forward.
  3. Pinch the nostrils closed with thumb and index finger. Place your mouth tightly over the person’s mouth. Use a mouthpiece if one is available. Blow two quick breaths and watch for the person’s chest to rise.
  4. Release the nostrils. Look for the person’s chest to fall as he or she exhales. Listen for the sounds of breathing. Feel for the person’s breath on your cheek. If the person does not start breathing on his or her own, repeat the procedure.



Once Ronan was done writing down the information The SunnFather Ziasla pulled out a ceramic life-sized doll with a movable jaw that really had only a head attached to a large bladder that looked like the stomach of a sheep. "Here, you can practice it on this dummy. Lets see if you can follow the directions." He said, looking around thoughtfully.

Ronan flopped back in his desk after the SunnFather informed him that they would investigate the contents of the packs. With a sunny smile he sat still as if he was in meditation, for that was about the only time he did. SunnFather Ziasla started to describe the reasoning for breathing, and the cether began to take notes once more.

He marked in his notes the issue of shock, and looked up in confussion. Which the SunnFather quickly responded that would be covered in the future. After he finished his notes he watched as the SunnFather removed the 'dummy'. Ronan look at it in curiosity, ready to attempt what was taught.

First he laid the dummy our flat, along a flat surface. Next he looked into the mouth, which he looked up of course there would not be anything in a dummy's mouth. But he stuck his small fingers into the mouth to 'clear' the airway.

Next was the breathing part, he tilted th head back and pinched the nostrils Wondering what one would do for a dracon? Then he placed his mouth onto the ceramic mouth and blew two times, then looked at the chest to see if it had risen. Then released the nostrils and placed his head on the dummy's chest listening for breathing. He then looked up for approval that he was doing it correctly, he would repeat the actions once more changing anything that the SunnFather may mention.

Had the SunnFather been overlooking Ronan's notes, he'd have been rather disappointed by the quality. But then again, the Cether people weren't normally known for their bookwork. They were doers rather than thinkers, and this quality shown forth when Ronan practiced on the dummy. He got it exactly right the first time. SunnFather Ziasla was a bit surprised. He'd expected the cether to be a miserable student, often gazing out the window, coloring pictures of daisy's on his notes, asking 'can you repeat the question' after one was asked of him. But it simply wasn't true. Ronan looked.... interested.

The SunnFather began to warm slightly.

"Now, lets talk about the second lifesaving step. It involves stopping bleeding. The first and most important treatment for an open wound is to stop the bleeding. Most bleeding can be controlled with a combination of direct pressure over the wound and elevation. Pressure gives the blood a chance to clot... thats making it thick and goopy... and elevation makes it harder for the heart to push the blood out of the wound because it has to go up hill."
He said, really going to the basics because it was in essence a basic class. At that point he went back to the chalkboard.




How to stop Mild Bleeding

Preparation
  • Wash hands thoroughly before beginning treatment.


Pressure and Elevation
  • Pack the opening of the wound with sterile gauze if the wound is large and wide.
  • Apply direct pressure to the wound by pressing with your hand and a piece of sterile gauze. If you don't have gauze, use folded fabric such as a bandanna.
  • Maintain steady pressure on the wound for 5 minutes, then release the pressure slowly and check for bleeding. Most bleeding will stop within 2 to 5 minutes.
  • Elevate the limb above the heart while applying pressure if the wound is on a limb and you don't suspect a fracture.
  • Layer additionals bandages on top of blood-soaked bandages. Don't remove the gauze that's already in place, as this may disrupt the clotting.
  • Apply pressure to pressure points if bleeding persists after 15 or 20 minutes of direct pressure.
Aftercare
  • Immobilize the limb using a splint, in cases of serious bleeding from limbs, to prevent bleeding from recurring.
  • Clean and dress the wound after bleeding has been controlled in order to prevent infection


On the next board he wrote some additional information and began the discussion. "There's a big difference between mild bleeding and severe bleeding. Mild is normally an annoyance, but severe bleeding can lead to death if the blood loss is significant enough." The healer said gently.




How to treat Severe Bleeding

Initial Assessment
  • Determine the location of the bleeding.

Elevation and Direct Pressure
  • Elevate the injured area above the heart.
  • Apply direct pressure to the bleeding area, using sterile cloth or gauze.
  • Keep the pressure on for five minutes.
  • Check to see if the bleeding has stopped. If it hasn't, apply pressure for 15 minutes.

Pressure Points
  • Check again to see if bleeding has stopped or significantly slowed. If it still hasn't, then locate the appropriate pressure point.
  • Find the brachial pressure point for bleeding on the arms: It's on the inside of the upper arm, just beneath the biceps, where a pulse can be felt.
  • Find the femoral pressure point for bleeding on the legs: It's in the soft area of the groin, at the crease between the inside of the upper leg and the crotch.
  • Press at the pressure point, toward the bone, to cut off blood flow.
  • Release pressure slowly after 10 minutes. If clotting has not stopped or significantly slowed the bleeding, and if professional help is far away, apply a tourniquet.
Extended Care
  • Clean and bandage the wound once bleeding has stopped or slowed to a trickle.
  • Splint serious wounds to avoid movement that could restart bleeding.



SunnFather Ziasla smiled. He noted the Cether was writing frantically and nodded. Good.. .he was paying attention.

"Finally, one last thing for this section. We need to know Tourniquets and Splits. If you look in your pack, you will find a few large square canvas cloths. Take one out, and we will go through that now. You can practice on your leg... but you must know that a tourniquet is dangerous. If a limb goes without blood for long enough, the person runs a risk of getting an infection and loosing that limb. So they are a careful balance of limiting blood loss and providing blood to an injured limb. Sometimes you might have to aply a tourniquet, loosen it, and reapply to provide blood to limb until you can get them stitched up. Thats an advanced healers technique."
He said, opting to work with the Tourniquet first.



How to use a Tourniquet
  • Determine if a tourniquet is necessary.
  • Gather the materials: a bandanna or cravat and a stick that won't break. If no sticks are available, use a trekking pole.
  • Fold the bandanna in half, from corner to corner; if using a cravat, don't perform this step. (The point is to form a right triangle, which cravats naturally are, whereas bandannas are square.)
  • Grab the corners that form the long side of the triangle and fold 3 to 4 inches toward the third corner.
  • Fold again and again in this manner until you've reached the third corner. You should now have a bandage 3 to 4 inches wide and several layers thick.
  • Tie the bandage with gauze around the appropriate limb between the wound and the heart, as close as possible to the wound but above the knee or elbow. Use an overhand knot (the same as the first stage in tying a shoe).
  • Place the stick on top of the knot and tie a second overhand knot over the stick.
  • Twist the stick until it pulls the bandage tight enough to stop the bleeding.
  • Tie the ends of the bandage around the limb and secure the stick with gauze.
  • Write "TK" on the injured person's forehead in grease pencil, along with the time of application.
  • Splint the wounded area to avoid movement, which could restart bleeding.




This was the part the instructor hated. "Any questions before we move on to splinting, wound care, and shock treatment?"

Bleeding, ugh. Thought the cotton topped cether, not that it totally repulsed him. For he had held little Reginald's sliced arm all the way back to his shire, after an unfortunate game of dare. Thier parents were both unhappy at was happened, but thanked Ronan for having the sense to rush his friend to the help. Ahh that must be what the SunnFather is talking about, for Ronan had held the boy's arm tight to stop the blood from gushing out.

But the talk of clotting, that brought a grimace to child like face of Ronan. But it was another bit that he took down into his new journal, one that he already felt would be his prized possession.

Ronan paused from writting, his little fingers had begun to cramp and he stretched them out. His teacher was still talking and mentioend the after care for both of the types of bleeding. So he quickly scribbled that portion down too... Wow this is alot of writing... It is like I am back at Schola under Taskmaster Havershamer, he thought with a scorned look on his face that was buried in the text he was writing.

Looking up Ronan thought he saw a faint smile on the SunnFather's face before he turned back to the chalk board. For some reason that made the young Cether more cheerful, for he was happier when other were happy. For some reason losts of folks seemed to be happier when he was around.

With a flurish he finished the last line and reashed into his fledgling healer's pack. He was slightly distracted by al that was in there, and was finding all sorts of interesting items when the set of large canvas clothes fell into his hand. He pulled on out and let the others drop back into the treasure chest of his kit. Following along with the teacher he folded the cloth to form a triangle, then folded the edge down two then three inches. WIth his bandage strap he looked up at the SunnFather with questioning eyes, he was to practice on his leg... but the one sentence he wrote flashed before his eyes.

He could lose a LIMB! So he was hesitant to follow through with the application of the folded strap, but slowly he lowered it to his thigh, his eyes darting back and forth between his leg and his teacher. After he tied a loose knot he looked up and nodded that he understood the principles of this technique, and was ready to move on. Before he removed the bandage his courosity had gotten to him, and he slid a wooden stick between his leg and the knot and began to twist. Ever so slightly till it clinched his leg... and a few moments later he felt the reduced blood flow. The tips of his proud toes began to tingle, done with the experiment be quickly remove the stick adn the knot. Soon he was wiggling and rubbing his toes with his hand, shaking his head no to the SunnFather's final question. The older man had been thoroughly thorough, which was plenty for the toe wiggling cether.

"So... now we have to work on splints. But first we have to figure out if something is broken. Any ideas on that?" The SunnFather asked. After they'd discussed Ronan's thoughts, the Monk would indeed outline another lesson written in chalk in the board.


How to Determine if Something is Broken
  • Talk to the injured person. Ask them about the nature of their accident, so you can see if there's a cause for a broken bone. Ask them what type of pain they are suffering. Fractured bones are usually very painful, though minor ones, (such as hairline fractures), may not cause any pain. However, this doesn't mean that they don't warrant prompt medical treatment, as they are still very serious injuries.
  • Gently touch the area in question, if it's a arm or a leg. Take note if you feel anything unusual, such as swelling or even the broken bone itself. If the affected area is in a more serious location, (such as the hips, the skull or the spine), do not touch it, as it could make matters worse. Besides, in these situations it becomes more obvious that something is wrong, since the person may not even be conscious.
  • Check to see how the body is responding to the affected area. The best way to do this is by feeling for a pulse. If there isn't a pulse, the victim may have a broken bone that requires thaumaturgic or advanced healer intervention.
  • If you suspect an arm or a leg is broken or if you are unsure, put a splint around it to keep it in place. The easiest splint for the arm can be created with medical tape and rolled-up newspaper. Wrap the newspaper around the arm and hold it in place with the tape. For the leg, attach it to something larger, such as a piece of cardboard. Keep in mind that if you don't have these materials on hand, you will have to use whatever is available to create your splint. This shouldn't be too difficult, if you remember the main components of a splint: 1) a firm surface and 2) something to help hold the splint in place.



SunnFather Ziasla smiled. Ronan was actually paying attention. Miracles never ceased to amaze him. Torek was being kind, that was for sure. "Now, that you understand how to figure out if something is broken... which comes more with time than anything else... lets look at how to make a split. Now you split differently for different parts of the body, but the most common breaks are arms and legs. These sorts of broken bones need splinting or casting for the entire time the leg or arm is mending." He said, speaking confidently. Then he turned back to his blackboard, erased, and began marking in chalk one more time.



How to splint an arm!
  • Determine if the arm is broken.
  • Immobilize the injured arm. Any movement will be very painful and can worsen the injury.
  • Get some rigid material long enough to extend from the elbow to the end of the hand. One possibility is to cut an Ensolite sleeping pad to the right length, and then fold it over to increase rigidity.
  • Gather materials to tie the splint into place. Any of the following will do: pack straps, rope, clothing, belts, bandannas, triangular bandages.
  • Flex the injured arm so that the forearm is at a 90-degree angle to the upper arm. Do this gently to minimize movement.
  • Put a wad of clothing or some other soft material into the injured person's hand to maintain it in a position of function.
  • Place the splinting material along the forearm, so that it extends at least from the elbow to the end of the hand.
  • Tie the splinting material into place below and above the fracture. Make sure the wrist is immobilized.
  • Place padding in any empty spaces between the splint and the arm.
  • Check circulation, sensation and motion beyond the fracture site, to make sure the splint isn't too tight.
  • Put the injured arm in a sling.
  • Continue with further treatment for the fracture.





Once that was done, Ronan was asked to get his splinting material and sling out of his medical kit, and a younger SunnSon by the name of Orias was brought in so Ronan could practice on. SunnFather was there to answer any of his questions.

Still working the blood back into his foot, he pondered the question the SunnFather posed. How can you tell if a bone is broke... It was a question he had never really thought of. With the first inklings of ideas he began to speak, 'I would imagine if it hangs funny... you know at a strange angle or something. Or if you can not use it, like walk on a leg or hold something up with your arm...' The thoughts finally finishing, his small hand went to his chin as he thought further.

As he was continuing to think of ideas, he saw the SunnFather rise and begin to write once more. A breath left his lungs as he moved to stand, and as he put weight on his test leg, he limped over to the desk that he had claimed as his own. From there he took his quill and began to take notes, though he tried to write down the instructor verbatim he was getting a little lax. He was as serious as ever in the actual instruction but thought that he could remember with fewer notes. Plus the more he practiced the better he would be, he thought. Less writing meant more practice.

As the instrutor turned and spoke to him, he lowered his quill and listened. This he was thinking was the longest that SunnFather Ziasla had spokent to him with out some remark on how Ronan 'should act'. It was refreshing for him and he proceded to give the insructor his full attention. Ronan tried not to let his gasping be heard as he lifted his quill once more. He understood the reasoning, for one day this journal would serve him in the field and he tried to make it complete. But he felt his hand would fall off, he wondered when that would be covered.

Place in sling, and further treatment if fracture.[/quote]With a smile on his face, he moved back to his Healer's Kit. He got out the needed materials to practice with. He was smiled as the young SunnSon entered, he would be able to practice with the instructor behind him. So he could focus on doing it correctly.

Following along with the role-playing scenario, he had Orias sit down. Ronan then asked him where it hurt and how bad it was. Moving his hands gingerly he probed the 'broken' area with his fingers to check the swelling and if he could tell a break. He made sure the young boy did not move and bent to gather the waddling and small sticks and the triangle bandages he used earlier.

Moving slowly and carefully, Ronan moved his 'patients' arm into an angled posiiton. From there he wrapped a rolled rigid section of the local newsprint to provide some support and padding. He then reached for the folded bandaged and folded the triangle several times till it was strap rather than triangle. With the arm padded, he binded the splingting material in place with the newly folded straps. He used a tight knot in two areas which secured the ends of the splinting sticks. But being sure to not pull the straps too tight to reduce circulation.

He stood and folded a sling, then placed it over the young boy's head. Then ease the 'broken' arm into place.

As all the items were completed Ronan looked proudly at the SunnSon Orias. Though he was not truely injured he hope he made good on his first attempt. To confirm that he stared up to the older SunnFather with large gray eyes, 'How was that SunnFather...'
